Continuum of Care Program (CoC)

(Formerly Shelter Plus Care Program)

The Continuum of Care Program provides rental assistance for hard to serve homeless persons with disabilities in connection with supportive services funded from sources outside the Housing Choice Voucher Program. Continuum of Care Program (CoC) is designed to provide housing and supportive services on a long term basis for homeless persons with disabilities and their families who are living in places not intended for human habitation (e.g., streets) or in emergency shelters. The program allows for a variety of housing choices as well as a range of supportive services in response to the needs of the hard to reach homeless population with disabilities.

The Longview Continuum of Care Program is a cooperative effort between three entities, the City of Longview, Community Healthcore MHMR and Special Health Resources of East Texas (SHRET). The Longview HA provides the rental assistance component of the program. Community Healthcore MHMR and SHRET provide the supportive services component. Interested applicants may apply for CoC through the appropriate supportive service agency: Community Healthcore for those with mental illness/drug abuse problems or SHRET for those with HIV and or AIDS. Once program eligibility is determined by the appropriate supportive service agency, applicants are then brought to the LHA for the rental assistance component of the program.
